You can have an annual health check if you are 14 years old or older and have a learning disability. An annual health check helps you to stay well by talking about your health and finding any problems early, so you get the right care.

How to book your annual health check
- First you need to contact your GP practice to ask to go on the GP's learning disability register.
- Ask your GP practice if you can book an appointment for an annual health check.
- Some doctors don't do health checks. If your doctor doesn't do them they should be able to tell you where you can have one and how to contact them.
