For each of the five inclusive approaches, you are asked a series of questions to evaluate and rate your practice and set out your support and action plan for the next 12 months. You are also asked to reflect on how the pillars of inclusive working underpin your work, and on your learning from applying the ICF.
Please note, in order to save the self-evaluation form during completion, you will need to have registered for a My Kirklees Account , or login if you already have one before starting the form.
Evaluating your progress
You will be asked the same questions for each Inclusive Approach.

- 3 The approach was considered and actions were developed to align with the approach. Evidence to measure impact/outcomes has been considered.
- 4 The approach has been largely aligned to, actions have been taken, although the approach is not yet fully embedded. There is some good evidence of outcomes/impact. There is a plan for continuous development.
- 5 The approach is in place and well established. There is strong evidence that the approach is having impact where needed. There is a commitment to continuous development.

What happens after you submit a self evaluation
It is advisable to download and retain the PDF version of your online submission, to act upon. This can be done once you have submitted your self-evaluation. All self-evaluations are submitted to the Communities Board for review and to support the development of the Communities Partnership Board ICF Implementation Plan.
The scores and support and action plans are an important measure of progress detailing what actions need to be taken to adopt the ICF approaches. In designing the self-evaluation tool, we have built in the important process of self-evaluation and learning. This will raise awareness and galvanise commitment to building inclusive communities. There should be as much importance placed on the quality and value of the self-reflective dialogue as the actual scores.
Each organisation has its own leadership structures, governance and impact measures, so would need to work through how they can use the tool to arrive at a high-level score as well as developing an implementation plan for their respective organisation.
